- Advertisements (4)
A copy of all advertisements that appeared in the four required venues:
(1) The Department of Labor / ETA requires a “print” advertisement that has been placed in a publication with national distribution for applicable professionals, for an unspecified amount of time. A copy of the page on which the advertisement posted is preferred, however; if the name of the publication and the publication date are not printed on the applicable page, the entire publication should be submitted;
(2) The UF Office of Academic Affairs requires departments and centers to advertise academic positions a minimum of 14 days on “GatorJobs” and in a minimum of two external venues (see UF Academic Affairs website for details).
- Notice of Job Availability
Document that posts an advertisement for the position and contact information for the regional Employment and Training Administration office in Atlanta, Georgia. If the college’s or center’s faculty are members of the faculty union (United Faculty of Florida – UFF), the University of Florida UFF President/Union Bargaining Representative must be provided with the Notice of Job Availability associated with the applicable ETA Form 9089. The Notice must be provided to the UF-UFF President/Union Bargaining Representative at least 30 days but no more than 180 days before the date the ETA Form 9089 is filed with the Employment and Training Administration. The Notice will also be posted on the UF website for ten business days, as required by the ETA. The sponsoring department/center must notify FSS of any inquiries that are associated with the applicable Job Availability Notice after the posting period ends.

- Diploma (Foreign national’s qualifying degree)
A copy of the degree that qualifies him/her for the applicable position (If the diploma is in a foreign language, please provide a translation, signed by the translator with a statement that addresses; (1) his or her competency to translate the document; (2) and the completeness and accuracy of the translation)
- Equity Compliance Report (if the position is a faculty position)
- Recruitment Report
A statement signed by the Department Chair, Center Director or Search Committee Chair (one who has actual hiring authority), outlining in detail the complete recruitment procedure, including: (1) the number of applicants; (2) number of applicants who did not meet the minimum requirements (if any); (3) a description of the recruitment steps undertaken and the results achieved; (4) the number of individuals who were offered the position, and their responses to the offer; and (5) the number of applicants who were rejected and the legitimate employment related reasons for this action.
- Offer of Employment Letter
Copy of current appointment letter (statement that addresses the employment eligibility issue must be included in all appointment letters to foreign nationals)
- Current I-94 card
Entry verification document that is attached to the foreign national’s passport
- Prevailing Wage Determination
Prevailing wages for positions in the State of Florida are obtained online at the website of the Agency for Workforce Innovation, Alien Labor Certification (SESA)
