HEALTH INSURANCE OPTIONS & INSTRUCTIONS

Insurance coverage for F-1 and J-1 students is mandatory . While proof of insurance is not mandatory for dependents of F-1 students, it is STRONGLY recommended that they also be insured as there is no medical assistance for international students or their family. Insurance for both J-1 students AND their J-2 dependents is a federal requirement. It is each student’s responsibility to maintain valid health insurance at all times. The Code of Federal Regulations states that if J-1 students and the J-2 dependents do not have Health and Accident Insurance coverage, the student’s program and visa status may be terminated.

You may purchase insurance for one or more semester(s) at a time. Make sure you have coverage for at least the current term for which you will be attending the university. You will be required to maintain coverage over the summer if you remain in the US, whether you are registered for classes or not.

The Insurance Verification Form must be filled out by a representative of your insurance company to verify that you have adequate insurance coverage if you purcharse a plan other than the UF sponsored plan.

AETNA Student Health Insurance - UF Sponsored Plan

As of August 14 th, 2008 the Florida State University System’s Board of Governors will require that all international students have health coverage during the entire year, including annual breaks. UF has a school–endorsed health plan offered through Aetna. The Student Health Center has staff to assist you with benefit explanations or claims.

GatorGradCare - UF Graduate Assistant Plan

University of Florida graduate students on an appointment as a graduate assistant, teaching assistant, or research assistant participate in the GatorGradCare Health Plan. To be eligible, University of Florida graduate students must be enrolled in a graduate degree program, on an appointment through University of Florida, appropriately registered and appointed .25 FTE (full-time Equivalent) or greater for a particular semester. University of Florida graduate students on a Fellowship are also eligible to participate. There is no FTE requirement for graduate students on a Fellowship appointment. Criteria for appropriate registration can be found in the Registration Requirements section of the Graduate Catalog. The contact at UF for assistance with enrollment, coverage and claims is Ms. Julie Neubig. She is located in the UF Human Resources office and can be reached at 352-392-0003.

The dates for the terms for which you may purchase insurance are as follows:

Fall: 8/16/2008-12/31/2008
Fall/Spring: 8/16/2008-5/15/2009
Spring: 01/01/2009-05/15/2009
Spring/Summer: 01/01/2009-08/15/2009
Summer: 05/16/2009-8/15/2009

Once you have purchased insurance, please provide UFIC with the requested receipt/confirmation of purchase (the dates for which you are covered MUST be visible). Make sure to include your UFID and Visa Type (F1 or J1) on the front of the proof of purchase.
